Forsyth County officials investigate nursing home after two residents reported missing

WINSTON-SALEM, N.C. — State health officials confirmed Wednesday that Forsyth County Department of Social Services officials are investigating two recent incidents involving residents at a Winston-Salem assisted living center.

Since March, two residents at Cornerstone Living Center have walked away from the center at 2900 Reynolds Park Road.

The latest occurred April 27 when Lewis Withers, 42, who has a mild mental impairment, walked away from the center. A Silver Alert was issued for Withers. Winston-Salem police found Withers on April 29 at an unspecified location and returned him to the center.
